We are so excited you have joined the CLPS family! You have made an excellent decision in spending more time with your student. Our goal is to communicate as much as possible about happenings in and out of school. This website will be your key to understanding what is happening at Christian Life Preparatory School. Please see below the Admissions Checklist. Please have all of these items completed by July 31st.

Immunizations

Every student must be current on all Immunizations to attend school or they MUST have an Immunization Affidavit on file from the state. You can submit immunization records digitally (email to [email protected]) or turn into the front office. Immunization Affidavits can be downloaded below and must be notarized and turned in to the front office. 

​Please Note - Your student may not be able to start school without the requested Immunizations or Affidavit on file prior to the first day of school.

Emergency Contact/Medical Release

​Please complete each online form below before July 31st. ​

Student Records

Submit Request for Student Records Form and/or Current High School Transcript for all students entering 1st to 12th grades (email to [email protected]). 

Family Photos

Please provide a passport like photo of each student and ​parent for us to use in the FACTS Family Portal (email to [email protected]).

  5. Attend all mandatory Parent Training meetings
    1. ​​​Parent Training for New and Current Families will be August 10-14, 2026. Mark your calendars now and try not to schedule other events on these dates. A more detailed schedule will be released this summer.​​​​​​​​​​​
